new-parent-questionnaire – Start the year on a positive note! *UPDATED* This is a product that provides teachers with valuable information on students and parents. Information provided in this questionnaire is brief but is not normally included in school administration paperwork. Parents have the opportunity to provide their own information (such as cultural background and hopes for their children). I have included different styles, questions and editable forms to suit most teachers needs. Parents, you can also print these forms and simply hand them in to your teacher if they were not provided to you. I am sure your classroom teacher will appreciate the extra information you have provided!

• standard forms to print and go (different styles included)
• editable answer forms (for parents to fill in online and email to return)
• some forms request parent cultural and religious information (only if parents wish to provide this)
• editable question forms – teachers can type their own questions for parents and print the forms to go!
